Admission Requirements

All residents are required to provide the following documentation upon admission:

  1. Social Security card
  2. Medicare card (if applicable)
  3. Additional insurance information or cards
  4. Copy of responsible party’s Power of Attorney (if applicable)
  5. Copy of Living Will or Health Care Power of Attorney (if applicable)
  6. Names and addresses of people to contact in an emergency

 

Admission from Hospital:

  1. In North Carolina, FL2 (with approval numbers) and PASARR
  2. In Kentucky, medical information and PASARR
  3. In Virginia, pre-screening UAI and PASARR
  4. MAR – Medical Administration Record*
  5. IV Administration Record (past 14 days)*
  6. Completed history and physical form*
  7. Chest x-ray or PPD
  8. Lab reports if applicable
  9. Therapy notes if applicable
  10. Signed discharge summary or transfer form with orders; signed by the physician.

* should be provided prior to admission

 

Admission from Home, Rest Home, Assisted Living, or Personal Care Facility:

  1. In North Carolina, FL2 (with approval numbers) and PASARR
  2. In Kentucky, medical information and PASARR
  3. In Virginia, pre-screening UAI and PASARR
  4. Completed history and physical form (within 5 days of admission)
  5. Signed physicians orders
  6. Chest x-ray or PPD (within 3 months)
  7. Home health notes (if applicable)

 

If admission is within two weeks of a hospital discharge and under Medicare Part A, requirements will follow under “Admission from Hospital.”

 

Admission from another Nursing Facility:

  1. In North Carolina, FL2 (with approval numbers) and PASARR
  2. In Kentucky, medical information and PASARR
  3. In Virginia, pre-screening UAI and PASARR
  4. Completed history and physical form (current or updated by physician)
  5. Lab reports if applicable
  6. Transfer form with orders signed by physician

 

If transferring under Medicare Part A, requirements will follow under “Admission from Hospital.”

Croatan Ridge Rehabilitation & Nursing Center admits residents without regard to race, creed or religion.

In North Carolina, please note that not all residents require an FL2, but all residents must have a PASARR-FMH number prior to admission to any facility. In Kentucky or Virginia, all prospective residents must have at least a Level I PASARR or a Level II PASARR (if applicable) prior to admission to any facility.

 

Coverage for Services

There are several options for payment of facility services. These include private resources and insurance as well as government resources, such as Medicare and Medicaid.

Private Insurance

Private insurance may cover short or long-term care depending on the provisions of the policy. Some plans will not cover room and board but pay for pharmaceutical or therapy costs. Please contact your insurance carrier to verify your benefits.

Private Resources

Private funds may be used to cover all costs associated with your stay. These costs include room and board, ancillary charges, and other services provided.

Medicare

Medicare is additional health insurance provided by the government. Medicare can pay up to 100 days per benefit period for skilled care in a nursing facility if certain coverage criteria are met. Coverage criteria includes a three consecutive night hospital stay and the need for skilled nursing or rehabilitative services. You may have more than one benefit period per year.

Medicaid

Medicaid, in conjunction with Social Security, is available to those who qualify financially and medically. Medicaid covers long-term nursing facility care. Applications must be placed with the Department of Social Services.
